The power of civic engagement is now in the palm of your hand; the city has launched the My Surprise App!

Graffiti? Uprooted/broken trees? Missing signs? Flooded roadways? Potholes? Irrigation/ landscaping maintenance? Broken street light? Records request?

The My Surprise app allows you to make service requests on these items and much more! You can even set your notifications to receive important community reminders, including a reminder for city-serviced sanitation pickup dates

Want to file a Records Request?

  • Click on the button below.
  • Once the page opens, click on “Service Requests”.
  • Then click on the map to designate Surprise City Hall, or type in 16000 N. Civic Center Drive, Surprise, AZ in the address bar and click “done” on far top right of page.
  • Select a report type: Public Records Requests or City Court Record Request
  • Complete the form.
