Medical
The City of Surprise offers eligible employees and dependents a choice of three comprehensive medical coverage options. All medical plans provide coverage for eligible expenses incurred for emergency care, medical services in the doctor’s office or hospital.
Pharmacy
The City of Surprise offers eligible employees a pharmacy plan through their medical plan options. All medical plans provide pharmacy coverage for prescription drugs.
Dental
The City of Surprise offers eligible employees and dependents a dental plan. The plan is designed to help eligible employees pay for a variety of dental services including Diagnostic/Preventive, Basic, Major and Orthodontic treatment.
Vision
The City of Surprise offers eligible employees and dependents a vision plan. Your vision care includes a comprehensive exam, glasses, and contact lenses.
Flexible Spending Accounts
Flexible Spending Accounts are available for both Health and Dependent Care Reimbursement. Contributing a Flexible Spending Account allows you to set aside money on a pre-tax basis to reimburse yourself for certain eligible expenses, thereby reducing your taxable income. You can use the money in your Flexible Spending Accounts to pay for qualifying dependent care expenses or for out-of-pocket health care expenses.
Group Life Insurance
The City of Surprise provides a basic amount of group Life and Accidental Death and Dismemberment (AD&D) Insurance to eligible employees who meet the active work requirement.
Additional Life Insurance
Life Insurance coverage is designed to help provide financial support and stability to your family should you pass away. Accidental Death and Dismemberment (AD&D) insurance provides an extra layer of protection if you die or become dismembered in an accident. You can also cover your eligible spouse and child(ren).
Employee Assistance Program (EAP)
Your EAP provides a wide range of services for all types of personal problems. Some issues frequently addressed and resolved by the EAP are family/relationship problems, parenting difficulties, work-related concerns and job stress, legal issues, bereavement, and alcohol and substance use.
Paid Medical Leave
All full-time regular employees are eligible for Paid Medical Leave (PML) upon hire. The wage continuation benefit for qualified reasons under this program are determined by the City’s contracted third-party administrator.
Aflac Supplemental Insurance
The City of Surprise has contracted with Aflac to provide additional benefits that specifically cover treatment for illness from cancer. Aflac provides a cash benefit for just about every part of the treatment regimen, from hospital confinement to radiation and chemotherapy.
