Membership is $12 per year for Surprise residents and $15 for non-residents. Fees are due with membership forms.

Facemasks are encouraged, but not required. Dates, capacity requirements and times are subject to change without notice.

For individuals age 60 years and better, and adults under 60 years old with a disability, the suggested contribution is $2.50 per meal. For individuals who do not qualify for the meal program, the suggested contribution is $5 per meal. It is highly encouraged to pay contributions using a credit/debit card.

If you are unable to participate in the in-person meal program, there is a home delivered meal option with Banner Olive Branch. For program qualifications and details, please call 602.264.4357.

Capacity for meals will be a maximum of 50 participants a day.

Beginning Monday, June 14, 2021, the fitness room will be open from 9:00 a.m. to 2:00 p.m. Participants are required to make a 45-minute appointment to schedule their workout.
