Passport services return to Surprise City Hall April 19

 Surprise, AZ (April 15, 2021)
Passport services will return to Surprise City Hall beginning April 19, by appointment only. 

Normal business hours for passports will be Monday – Friday, 8 a.m. to 4 p.m. in the City Clerk’s Office. 

City staff can accept applications for first-time passport books, passport cards and can assist with reviewing paperwork for renewals. This includes adult, adolescent and child passports. Please make sure your passport applications are completed in black ink only before arriving for your scheduled appointment.  

The City of Surprise does not offer passport photos. Please make sure you have your photos with you when you arrive for your passport appointment.

Any fees paid to the Department of State must be paid by personal check or money order. Fees paid to the City of Surprise for Execution Fees can be made by personal check, cash or debit/credit card.
