Are Filing An Unemployment Insurance Claim?
Claimants may file Unemployment Insurance Claims using any one of the following methods:
Online: To file for unemployment insurance benefits online, visit www.azui.gov and click on the button titled “Apply for UI Benefits.”
Paper: The Initial Claim form for Unemployment Insurance is on the website and can be printed and submitted by fax or mail. https://des.az.gov/sites/default/files/dl/UB-105.pdf
