The City Clerk’s Office at Surprise City Hall offers passport services
Monday – Friday, 8 a.m. to 4 p.m.
is located at
16000 N. Civic Center Plaza Surprise, AZ 85374.
Services are conducted on a first come first served basis, appointments are not available.
We accept applications for first-time passport books, passport cards and can assist with reviewing paperwork for renewals. This includes adult, adolescent and child passports.
The City of Surprise does not offer passport photos, please make sure you have your photos with you when you arrive for your passport appointment.
Fees paid to the Department of State must be paid by check or money order
Fees paid to the City of Surprise for Executive Fees can be made by check, cash or debit/credit card
