The City Clerk’s Office at Surprise City Hall offers passport services Monday – Friday, 8 a.m. to 4 p.m. City Hall is located at 16000 N. Civic Center Plaza Surprise, AZ 85374.
Effective April 2, 2018, the passport execution fee will increase from $25 to $35. The $10 execution fee increase only applies to U.S. passport applicants using the DS-11 form, such as first-time applicants over 16, children under 16, and applicants who re-apply after reporting their previous passport lost or stolen. Services are conducted on a first come first served basis, appointments are not available.
We accept applications for first-time passport books, passport cards and can assist with reviewing paperwork for renewals. This includes adult, adolescent and child passports.
- Fees paid to the Department of State must be paid by check or money order
- Fees paid to the City of Surprise for Executive Fees can be made by check, cash or debit/credit card
