The City of Surprise is temporarily suspending Passport Services in response to the rise in COVID-19 cases. To help protect the health and safety of staff and customers, we are canceling all appointments we are canceling all appointments effective January 6 through January 21, 2022.

The City sincerely apologizes for this inconvenience. You may rebook your appointment using our online system for a time on or after January 24, 2022.

The City Clerk’s Office at Surprise City Hall offers passport services Monday – Friday, 8 a.m. to 4 p.m. by appointment only.

We accept applications for first-time passport books, passport cards and can assist with reviewing paperwork for renewals. This includes adult, adolescent and child passports. Please make sure your passport applications are completed in black ink only before arriving for your scheduled appointment.

Note: The City of Surprise does not offer passport photos, please make sure you have your photos with you when you arrive for your passport appointment.

Payment information:

  • Fees paid to the Department of State must be paid by personal check or money order
  • Fees paid to the City of Surprise for Execution Fees can be made by personal check, cash or debit/credit card
