  1. Medical Plan Information

    The City of Surprise offers eligible employees a choice of two comprehensive coverage options through Blue Cross and Blue Shield of Arizona. Eligible employees can enroll in the Blue Preferred plan (PPO) or the Blue Choice plan (HMO). Both medical plans provide coverage for eligible expenses incurred for emergency care, medical services in the doctor’s office or hospital, and prescription drugs.

  2. Dental Plan Information

    The City of Surprise’s dental plan is offered through Total Dental Administrators (TDA). The plan is designed to help eligible employees pay for a variety of dental services including Diagnostic/Preventive, Basic, Major and Orthodontic treatment. The dental plan is a Preferred Provider Organization (PPO) which allows you to receive coverage through both In-Network and Out-of Network providers.

  3. Vision Plan Information

    You and your eligible dependents are covered by Avesis for your vision care needs including glasses and contact lenses. Avesis offers a PPO program which allows for coverage at both In-Network and Out-of Network providers.

  4. Group Life Insurance

    The City of Surprise provides a basic amount of group Life and Accidental Death and Dismemberment (AD&D) Insurance through Standard Insurance Company to eligible employees who meet the active work requirement.

  5. Post Employment Health Plan

    All benefit eligible City employees participate in a Post-Employment Health Plan (PEHP). The PEHP is a benefit designed to allow the City and employees to set aside money for the payment of qualified medical expenses incurred post employment.

  6. Employee Assistance Program (EAP)

    Your EAP provides a wide range of services for all types of personal problems. Some issues frequently addressed and resolved by the EAP are family/relationship problems, parenting difficulties, work-related concerns and job stress, legal issues, bereavement, and alcohol and substance use.
