Procedure for submitting a Citizen Action Request
The Citizen Action Request form should be used by citizens to submit their comments or concerns about the Police Department or an employee. Return the completed form in person or by mail to the:

Once the form is received, should further investigation be necessary, you may be contacted for additional information. You will be contacted regarding the outcome of an investigation.
Motor Vehicle Accident Reports
Starting January 14, 2008, redacted motor vehicle accident reports written by the Surprise Police Department, will be available on-line. Click here http://surpriseaz.policereports.us/ for detailed information.
