Procedure for submitting a Reviewable Action Form
The Reviewable Action Form should be used by citizens to submit their comments, concerns or commendations about the Police Department or an employee. Return the completed form in person or by mail to:

Once the form is received, you may be contacted for additional information if further investigation is necessary. You will be contacted regarding the outcome of an investigation.
